Orchis simia is an odd flower with a lip showing narrow and tortuous lobes, dark purple at the extremities, that recalls a monkey figure.

An appendix is localized between the two lobes.

Contrary to O. militaris, when blooming top flowers open first. The bright green leaves are not spotted.

Flowering season lasts from April to June.

It frequently forms hybrids with other Orchis.
